Post-Grant Feedback Form
The Duxbury Education Foundation requests grant recipients submit feedback about the grant process within two months of the grant's implementation to assist DEF in improving the grant process. Also, DEF requests at least one photograph of the grant in action to include in the Duxbury Education Foundation Annual Report, on the website and/or on social media sites. Photographs should be full-size jpeg files.
Before submitting photographs, please confirm that any pictures showing students are approved by the school to be used in DEF's social media and marketing materials.

- Contact Name
- Grant Title
- When was your grant implemented (month/year)?
- Summarize the impact of the grant and its intended beneficiaries.
- What worked well with the implementation of the grant? What didn't go as expected?
- How will this grant continue to be utilized in the future?
- Rate the grant submission process (1=best to 5 = worst). How can we improve?
- Rate the grant fulfillment process (1= best to 5= worst). How can we improve?
- Would you recommend pursuing a DEF grant to others?
