Post-Grant Evaluation Form

The Duxbury Education Foundation requests grant recipients provide a post-grant evaluation plus at least one photograph within 2 months of implementing the grant. DEF may include images in our Annual Report, on our website and on social media sites. Photographs should be full-size jpeg files and approved by the school if students are in the photograph.
